PENNSYLVANIA CHILD ABUSE HISTORY CLEARANCE

Obtain a Child Abuse History Clearance from the Department of Human Services: You can complete this online at https://www.compass.state.pa.us/CWIS. You will need to create an individual account, which could take a day or two to process. Once you have an active account, follow the online instructions to complete the clearance request. This clearance is free for volunteers every 60 months. Once the report is complete, please print a copy and return it to the Associate Pastor. Please make a second copy for your own records. All records are kept confidential.

NOTE: To complete this form, you will need to obtain to the best of your ability all the addresses where you have lived and names of residential roommates you have had since 1975, including children, spouses, and parents. Reports are valid for 60 months, after which time it is the responsibility of the volunteer/employee to obtain updated clearances.

This clearance must be obtained through the state.

If you have completed this clearance for another organization, you can make a copy of that report and submit it to Calvary. Reports are valid for 60 months. If you believe you have already submitted the Child Abuse History Clearance to Calvary, please verify with the Associate Pastor.
